Adult Mental Health Programs & Services

Joseph Brant Hospital's Community Mental Health services provide a continuum of care from assessment or consultation, to specialized treatment supports for clients with complex and serious mental health disorders.

General Psychiatrist Consultation Clinic 

The Psychiatrist Consultation Service provides assessment, medication review, and treatment recommendations via a "shared care" model with family physicians. 

Exclusionary criteria (as primary cause for referral): Autism assessments, Eating Disorders, Dementia, Gender Dysphoria. Assessments are not given for the purpose of ODSP, WSIB, forensics or CAS

 Short-Term Treatment

Provides Short Term individual therapy for adults experiencing predominant mood & anxiety disorders; complex personality disorders with comorbidity, and severe adjustment reaction disorders where group therapy is not evidenced based.

  • Adults 18 and over
  • Time limited, average of 12 therapy sessions
  • Use of DBT, CBT, Solution Focused, and Interpersonal therapies

Dialectical Behavioral Therapy (DBT)

Two formats of DBT are offered, the Comprehensive Program and the DBT Skills Only Group. 

  • Comprehensive Program: Offered to clients diagnosed with Borderline Personality Disorder. Intensive individual and group treatment lasting 26 weeks.
  • DBT Skills Only Group: Group based intervention to assist individuals diagnosed with Borderline Personality Disorder/Traits or who have difficulty regulating emotional responses. Skills are taught to help clients better manage behavioural, emotional and cognitive instability through Mindfulness, Distress Tolerance, Emotion Regulation & Interpersonal Effectiveness

 

Concurrent Disorders Clinic 

Provides individual and group assessment, consultation and treatment for individuals with co-occurring mental health and addiction disorders. 

Intensive Case Management

Intensive Case Management Services offers comprehensive outreach services to adult clients and their families. 

This program addresses the multiple needs of clients with predominantly psychotic or severe mood disorders (e.g. bipolar) and focuses on the client's goals and effective community reintegration.  

Medication Clinic

The Medication Clinic facilitates the optimal use of psychotropic medications to ensure client stability in the community and supports clients in maintaining adequate functioning and an enhanced quality of life. Primarily for individuals on an injectable medication.

Phoenix Program-Early Intervention Service 

The Phoenix Early Intervention in Psychosis Program services individuals aged 14-35 years old who are experiencing early signs and symptoms of psychosis. This recovery-focused program helps individuals achieve their goals at work, at school and within their social networks, thereby reducing the impact psychosis can have on one’s life. The team includes psychiatrists, nurses, a social worker, an occupational therapist, a peer support worker and a substance use counsellor.

  • Psychiatric assessment and treatment
  • Medication and symptom management
  • Case management
  • Education about symptoms and treatments
  • Relapse prevention planning and strategies
  • Family education and support
  • Substance use counselling
  • Peer support
  • Assistance with education and work
  • Assistance with connecting to other community agencies

Accessing Care

People can self-refer or can be referred by a health professional or physician. 

Referrals

Referrals are accepted from physicians, hospitals, community agencies, and mental health and addiction professionals. There is an intake review to determine eligibility for all treatment services.

Centralized Adult Mental Health & Addiction Services Referral

 All referrals must have a completed PHQ-9 and GAD-7

Location: Joseph Brant Community Health Centre (JBCHC), next to the hospital 1182 North Shore Boulevard (east door entrance located just off North Shore Boulevard ) Burlington, ON Community Mental Health is located on the lower level of the Joseph Brant Community Health Centre.
