The Hamilton, Niagara, Haldimand and Brant Community Care Access Centre (CCAC) helps people of all ages return to their homes following surgery or a hospital stay. The CCAC provides information about community supports and services, arranges government-funded home care services, and facilitates placement into long-term care homes such as nursing homes.
The CCAC helps people get the services they need to reach their health goal, be it:
. Staying at home
. Avoiding a hospital stay or;
. Getting support when you leave the hospital
The CCAC knows the community well and can recommend services such as:
. Meal programs
. Support groups and;
. Other resources to help individuals live comfortably at home
Some services or programs in the community may have costs associated with them.
A physician's referral is not needed to access CCAC services.

Patient Transfer Ontario is a non-emergency patient transportation system.
The base fee for use via Ontario Patient Transfer is $165.55 for a 10km distance. For more information call toll free 1-866-527-9191.
